DES MOINES, IOWA – Agri Star Meat & Poultry plant in Postville, Iowa, was fined $28,415 and given four citations by Iowa’s Occupational Safety and Health Administration (OSHA).

The facility was the site of a February explosion that injured two employees. 

OSHA records revealed that ice and debris were on the dock at the plant on Feb. 15, which caused an employee to back a forklift onto a propane tank leading to an explosion.

The largest of the fines handed out by Iowa OSHA was $11,366 for the various ice, dirt, water and turkey manure on surfaces. Employers must keep walking and working surfaces free of hazards for employees, according to the Iowa OSHA report. 

In another citation was a $9,472 penalty. Iowa OSHA said Agri Star failed to ensure powered industrial truck operators slow down on wet and slippery floors. 

Additionally, OSHA explained that Agri Star did not conduct a performance evaluation of industrial truck operators at least once in three years. This resulted in a $7,577 fine.

According to reports Agri Star paid the penalties laid out by the agency.
