PENNSDALE, Pa. – The Pennsylvania worker who was killed after falling into a meat grinder has been identified as Jill Greninger, 35.

Greninger was operating the grinder when she fell or was pulled into the machine. There were no witnesses to the accident, according to local authorities. The Lycoming County Coroner’s Office reported that Greninger may have been standing on a set of wheeled stairs before the accident. A co-worker found Greninger after hearing strange noises coming from the machine.

The federal Occupational Safety and Health Administration (OSHA) is investigating the incident.
